Focus on comfort and the individual situation
If your dog is still living, ask the veterinary team about pain, breathing, ability to eat and drink, mobility and the expected course of the condition. Separate observations from conclusions
A dated record can distinguish what happened from what it might mean. Note a familiar activity your dog could or could not manage, signs of discomfort and questions for the clinician. Do not use an online score or a fixed countdown to decide that death is inevitable or that treatment is unnecessary.
If the loss has already happened
You may want to write down a memory or ask the veterinary team to explain what occurred. You do not need to resolve what your dog understood in order to acknowledge your grief. The note sheet is optional and can be kept for yourself rather than used as a medical record.
