Some questions need a conversation rather than a calculation. Use this sheet to collect observations, memories, practical questions or follow-up steps before speaking with someone who knows your pet.
Make observations concrete
You might record when something began, what changed, how often it happened and what was different from your pet’s usual behaviour. Describe what you noticed without needing to give it a diagnosis.
Keep your questions together
Write down what you want to understand, what information you need to bring and what you would like to do next. The sheet can also hold memories or questions about a difficult experience; it does not assume that every situation needs a feeding or exercise plan.
Save it in a way that works for you
Download a CSV to keep the date and both text fields together. Line breaks, commas and quotation marks remain part of your note. Or copy the sheet into a document, a message you choose to send, or a new ChatGPT conversation.

Does this check symptoms?
No. It organises what you enter and does not interpret severity, suggest medication or recommend a diagnosis. For an urgent problem, contact a vet instead of completing a note sheet first.
