Some questions need a conversation rather than a calculation. Use this sheet to collect observations, memories, practical questions or follow-up steps before speaking with someone who knows your pet.

Make observations concrete

You might record when something began, what changed, how often it happened and what was different from your pet’s usual behaviour. Describe what you noticed without needing to give it a diagnosis.

Keep your questions together

Write down what you want to understand, what information you need to bring and what you would like to do next. The sheet can also hold memories or questions about a difficult experience; it does not assume that every situation needs a feeding or exercise plan.

Save it in a way that works for you
